Dexamethasone is a potent synthetic member of the glucocorticoid class of steroid drugs. It acts as an anti-inflammatory and immunosuppressant. It is 20 to 30 times more potent than the naturally occurring hormone cortisol and 4 to 5 times more potent than prednisone.

This video shows you intratympanic drug delivery to the inner ear. Our protocol takes place in the outpatient clinic under mi-croscopic control. The patient lies flat on a stretcher in the office with the neck fully extended and the head turned 45 degrees to the opposite side. We use Emla cream (Lidocaine 2.5%/Prilocaine 2.5%, Astra Zeneca Pharmaceuticals, LP, Wilmington, DE) to regionally anesthetize the tympanic membrane in the quadrant anterosuperior of the involved ear. Either the dexamethasone solution, gentamicine or hydrogel are loaded into a 3-mL syringe on which is placed a 22-gauge needle. The tip of the needle is angled. Filling the middle ear with a simple needle injection was not difficult. After the injection was administered slowly, the patient was instructed to lie in the supine position with the treated ear up for one hour, to keep their head still, and to swallow as little as possible, with no talking. Subsequent injections were delivered in the same way each day for the next 4 days. After each day's procedure the patient returned to his or her normal daily activities.

Here is my video update. My doctor prescribed me dexamethasone to lower my androgen levels. I'm nervous to start the medication but I'm hopeful that it will help me greatly.
